Hospice Care
Chicago Hope and Palliative Healthcare believes that when managing a terminal illness, there is no place like home. Wherever home is for you, Chicago Hope and Palliative Healthcare is the expert in bringing you the comfort, support, and care you need where you are most comfortable. While surrounded by loving family members and familiar things, we bring peace of mind in knowing that your last months, weeks, and hours are pain-free, safe, and comfortable.
Home care is provided wherever you call home – a private residence, nursing facility, or residential care facility. Our expert hospice staff are available to assist 24 hours a day. With education and support, you are in control of your health care. Our goal is to assist whenever needed and work closely with you and your family to bring the level of care that you and your hospice team decide is most appropriate.
Our physicians and nurse practitioners make house calls as needed and work closely with your hospice team of nurses, social workers, aides, spiritual care coordinators, and volunteers to ensure that your needs are met. Our Home Care program allows for treatment to manage pain and symptoms, assistance with patient meals, and light household duties in the patient area, as well as additional therapies if needed.
Continuous Care is a short-term higher level of nursing care to provide relief of acute pain and symptoms in the home setting. If a more intensive level of care is needed, Chicago Hope and Palliative Healthcare has three inpatient facilities that may be used if the physician and hospice team feel this is the best option for the patient and family.
The goal of hospice care is to provide the best possible level of care in the most appropriate setting, which allows the patient and family to spend quality time together. A plan of care will be developed and closely followed to ensure that this goal is honored.
